The problem
Odoo Survey collects text, numbers, and choices, but it cannot collect a file. Claims, HR, quality, and admissions teams that need a photo, a PDF, or a signed form attached to a response are forced into a separate email inbox, a third-party form builder, or a manual follow-up — leaving the attachment disconnected from the survey record it belongs to.
The solution
Two new question types — File Upload and Signature — let respondents attach images, documents, or a drawn/typed/uploaded signature directly inside the survey form. Every file lands inside Odoo, linked to the exact answer, with per-question size, count, and file-type rules, plus a reviewer experience built for browsing and bulk-downloading many responses at once.

File Upload question type
A first-class question type in the survey builder that accepts images, PDFs, and any document type — one question for every media kind instead of stacking single-purpose apps.
Signature capture
A dedicated Signature question with three input modes — freehand draw, typed name, or image upload — and a preview before the respondent completes the survey.
Single or multiple files per question
Configure a question for exactly one file — a signed form, a single photo — or toggle multiple uploads with a per-question maximum file count, such as several damage photos on one claim.
Size limit and file-type whitelist
Set a maximum file size in MB and restrict accepted extensions per question (PDF only, images only, or any custom list) so uploads always match the process, not the respondent's guess.
Instant client-side validation
Oversized files, disallowed types, and duplicate uploads are flagged in the browser before submission, with a specific message stating exactly which rule was violated — not a failed upload after the fact.
Duplicate detection and removal before submit
Uploading the same file twice for one question is detected and blocked automatically, and respondents can preview and remove any staged file before they finish the survey.
Drag-and-drop, mobile-responsive drop-zone
A touch-friendly drop-zone with thumbnail previews works the same on desktop and mobile, matching where most survey respondents actually answer.
Reviewer thumbnails, smart button, and per-response zip
Every response gets an "Uploaded Files" smart button and page with image thumbnails, plus a one-click "Download all files" action that zips every attachment for that response.
Dedicated Uploaded Files list and kanban
A standalone list and kanban view lets reviewers browse, search, and filter every uploaded file across all surveys by survey, question, respondent, image type, and signature — built for processing many responses at once, plus a per-question bulk download.
How it works
Add the question
Pick File Upload or Signature in the survey builder and set the size, count, and file-type rules on the Options page.
Respondent uploads
The respondent drags a file or draws/types/uploads a signature; each file is validated and previewed instantly in the browser.
Files attach on submit
Submitting the survey links every uploaded file to its answer, so the response record is complete and self-contained.
Review and download
Reviewers open the response, see thumbnails on the Uploaded Files page, and download one file or the whole response as a zip.
