How it works
Capture the referral
Create a case one at a time, or generate a cohort in bulk from a selection of appointments. Each case gets an automatic reference and a linked patient.
Plan and confirm
Record the reason for referral and community service, set contacts planned and an assessment template, then move the case to Confirmed.
Deliver and assess
Log contacts made as they happen and capture before/after wellbeing scores against the configured assessment questions, then mark the case Done.
Review and report
Open the coordinator dashboard for aggregate outcomes, then export or print the Referral Outcomes report for commissioners and funders.
